Up to $90,265 Annual Salary $5,000 Relocation assistance Up to

RESPONSIBILITIES AND FACILITY DETAILS Licensed Clinical Social
Worker (LCSW) Integrated healthcare model - FQHC Monday through
Friday, 8am to 5pm, Optional Over-time hours available
Collaborative Environment - working with Primary Care to find the
best solution and treatment plan that addresses all areas
QUALIFICATIONS AND SKILLS Licensed Clinical Social Worker (LCSW)
Bilingual / Spanish-speaking candidates welcome (not mandatory) New
