SUMMARY AND PURPOSE:Responsible for providing comprehensive
medication management services for patients cared for by the Saint
Alphonsus Medical Group (SAMG) Clinics and/or patients managed by
the Saint Alphonsus Health Alliance (SAHA). - Works collaboratively
with physicians, advanced practice providers, staff and other
health care professionals to provide medication management,
education, and care coordination across the health care continuum.
-Focuses on supporting patients upon discharge from an acute care
and post acute care setting and providing chronic disease state
management of high and rising risk patients associated with
assigned Saint Alphonsus Medical Group Clinics, through
collaborative practice agreements, and with patients of practices
in the Saint Alphonsus Health Alliance under the supervision of the
Executive Medical Director Value Care. -Is an integral member of
the health care team who works to ensure that excellent patient
experience, patient safety, best practices and high quality
standards of care are maintained across the continuum of
care.Responsibilities include assisting the Saint Alphonsus Medical
Group Clinic and Alliance network providers and staff in developing
and coordinating a wide range of self-management support and
chronic-care management activities for the clinic's affected
patient population.The focus will be on patients that have complex
medical needs, are prescribed extensive medication regimens,
endorse barriers to adherence and medication access, require
additional educational needs and would benefit from clinical
pharmacist follow up between primary care of specialty provider
visits.Success will be measured by the results of the process and
outcome performance measures around patient experience, reducing
readmissions and unnecessary hospitalizations of the assigned SAMG
clinic and SAHA patient population. -Supports the overall efforts
of care coordination and population health management teams across
the healthcare system.SKILLS, KNOWLEDGE, EDUCATION AND
EXPERIENCE:Currently licensed in good standing with the Board of
Pharmacy in Idaho required; licensed in Oregon required within 6
months of hire. PharmD required. - Bachelor's Degree in Pharmacy
and 3 years of experience may be considered in lieu of PharmD.
Minimum of 1 year of practice experience at patient care level
required or equivalent. Residency training: Post Graduate Year 1
preferred.Board Certification in Pharmacotherapy or Ambulatory Care
preferred. Previous experience in both acute care and community or
long-term care pharmacy preferred.ESSENTIAL FUNCTIONS:Knows,

boundaries with patients/families. Understands the expectations of
payers that contract with SAMG or SAHA and supports compliance with
these payer's pharmaceutical management programs and quality
measures.Communication: Monitors therapy for appropriateness,
dosage, effectiveness, interactions, and adverse drug reactions.
Works with health care team to optimize pharmacotherapy
opportunities, formulary adherence, reduction in poly pharmacy and
associated drug cost. Demonstrates knowledge of clinical paths
(e.g., stroke path) and order sets. Reviews and acts upon automated
surveillance and clinical decision support tools. Documents
suspected adverse drug events, interventions, QAR's, and initiates
other organizational and/or departmental required documentation.
Assists with medication utilization evaluations. Provides feedback
related to clinical pharmacy risk issues and clinical pharmacy risk
reduction. Seeks feedback from staff, physicians, and other leaders
and shares concerns appropriately.Compliance: Supports department
and system-wide requirements for compliance with Saint Alphonsus
Health System policies, regulatory and accreditation-related
programs, and quality/performance improvement efforts within the
organization. Successfully completes, in required timeframes,
annual competency requirements to support training and education,
accreditation, and licensure. Actions are consistently directed
toward and aligned with departmental and organizational mission,
goals, and objectives, departmental initiatives and interests in
the delivery of services, and the fulfillment of job

time.Provides the highest and most remarkable quality of service by
actively supporting, demonstrating, or participating in community
health education opportunities Provides guidance and direction for
patient education and medication safety. Patient Assessment:
Identifies high risk patients as defined by the evidence-based
literature or from internal databases including patients with
complex medication history, multiple chronic conditions, and or
identified as having suffered complications of an adverse drug
event. Review identified patients' medications and clinical
conditions for complication-prone regimens, redundant medications,
overt contraindicated medications and allergies, or complicating
substances such as herbal products and supplements and make
recommendations, as appropriate, to the care provider or associated
specialists for any quality or safety interventions.Order lab work
per Collaborative Practice Agreement to monitor safety and efficacy
of medications prescribed. Ensures optimal medication therapy
choice based on patient characteristics including concomitant
medications, medication cost, previously documented
ADEs/intolerances, -and clinical conditions as per evidence-based
clinical practice guidelines. Makes recommendations to the
appropriate provider if applicable. Completes medication
reconciliation of list within E.H.R. Patient Education: Educates
identified patients on the management of their disease states and
appropriate medication use with emphasis on adherence and
identifying/resolving medication related problems.Enhances
communication between acute and ambulatory care providers and
between specialty care providers when multiple providers may be
involved in a patient's care. Provides both verbal and written
educational materials depending on patient preference in keeping
with appropriate health literacy. Utilizes tools such as Ask Me 3
and Teach Back to ensure patient understanding.Staff Development:
Facilitates learning experiences to meet identified learning needs.
Supports staff development through sharing of
information/educational resources. Educates staff associated with
the Saint Alphonsus Medical Groups on medication related issues.
Provides medication updates through electronic methods, newsletters
and presentations to providers and staff as applicable.Leadership:
Provides guidance and direction related to professional practice.
Provides feedback and support to all members of the care team which
may include: CMA, Pharmacy Technician, pharmacy residents, and
Pharmacy Students. Accepts responsibility for positions supervised
and includes orienting, training, evaluation and disciplinary
counseling.Continuous Improvement: Supports the development and
implementation of policies and procedures across the system
consistent with practice and regulatory requirements. Serves as a
resource and consultant for improvement efforts. Supports and
facilitates process improvement in the organization and across the
system. Participates in improvement efforts. Professionalism:

staff and other healthcare professionals.Participates in
