seeking a reliable and compassionate Hospice Clinical Services
Manager to help lead our team! Summary: The Community Liaison is
responsible for managing community partnerships by educating
facility staff about Bluebird Health's services. This position
should be someone who is a people-person, driven, and has a passion
to help connect people with Idaho's premier Home Care, Home Health,
and Hospice company. The Community Liaison is also responsible for
driving business growth for Bluebird Health. This position is meant
to be our face in the community. Schedule: Full Time, Exempt
Education:
- Bachelors Degree PreferredLicenses/Certification:
- Licensed drive with an insured automobile in good working
orderExperience:
- Community engagement/marketing/sales experience preferred
- Healthcare experience preferred, but not requiredWhat you can

- Educate health care professionals, patients, and families about
hospice services
- Develop and maintain relationships with key referral sources,
such as hospitals, nursing homes, and physician offices
- Attend community events to promote hospice awareness
- Serve as a resource for patients, families, and caregivers
during the hospice journey
- Coordinate patient admissions with the interdisciplinary
team
- Participate in patient care conferences and develop
individualized plans of care
- Conduct on-site visits to assess patient needs and identify
barriers to care
- Monitor patient progress and revise plans of care as
needed
- Advocate for patients and families to ensure they receive the
best possible care
- Keep accurate records of patient interactions and
referrals
- Attend continuing education courses to keep abreast of changes
in the hospice industry
- Proven track record of meeting sales goals and targets
- Excellent communication, presentation, and negotiation
skills
- Self-motivated with strong time-management skills
- Ability to work independently as well as part of a team
