Job Description:
Saint Alphonsus Health System is seeking a full-time Nurse
Practitioner or Physician Assistant specialized in geriatrics to
join our outpatient team in Nampa. This role is pivotal in
delivering exceptional care tailored to the elderly population,
aiding in chronic illness management, preventive care, and
enhancing overall patient well-being. Our focus is on a
collaborative, holistic approach that aligns with innovative
practices to optimize health outcomes. The candidate will be part
of a multidisciplinary team dedicated to improving patients' health
and quality of life in a community renowned for its commitment to
personal wellness and medical excellence. Conduct comprehensive
assessments of geriatric patients to evaluate health status and
identify treatment needs. Develop, implement, and monitor
individualized care plans in collaboration with a multidisciplinary
team. Manage chronic diseases and advise on preventive care
measures. Educate patients and their families on health maintenance
and disease prevention. Coordinate with healthcare providers to
streamline care and ensure patient coverage across settings.
Participate in ongoing professional development and contribute to
team meetings to discuss patient care and clinic operations.
Advanced practice clinical skills in geriatric care Excellent
communication and interpersonal abilities Proficient in electronic
health record systems Strong decision-making skills and
professionalism Ability to work collaboratively in a team-oriented
