We truly find joy through service.We are seeking a full time Hospice Admissions Nurse to join our team.Summary:The Hospice Admissions Nurse provides services to patients to admit said patients onto hospice services. Duties can be strictly admissions or admissions and providing visits - depending on hours requested.Schedule:Monday through Friday. Hourly or Salary depending on schedule and duties preferred. Competitive pay DOE.Education:Graduate from an accredited School of NursingLicenses/Certification:Current RN License (ID or Compact.)Current BLS certification or willingness to obtainLicensed drive with an insured automobile in good working orderExperience:1+ years RN experience. Contact thendrickson@bluebirdhealth.comRequirementsRESPONSIBILITIESAdmit patients onto hospice services through approved Medicare medical diagnosis.Coordinates hospice care by working with other team members as well as with the patients/family members to ensure needs are met.Evaluate the effectiveness of hospice services to the patient and family on an ongoing basis.Perform evaluations for hospice care along with informational visits as needed - depending on hours chosen for strictly admissions or not.Utilize the nursing process to provide professional nursing care.Coordinate patient services with other members of the IDG as well as contracted services, when needed.Participate in IDG and case conferences.Provide education to patients/family members regarding the dying process, plan of care and offers guidance with healthcare decisions.Adhere to Federal, state and accreditation requirements including Medicare and Medicaid regulations.Salary Description$40-$44 Compensation DOEMedical SpecialtyMedical Specialty was not provided and will not appear on the job post.AddPhysical SettingPhysical Setting was not provided and will not appear on the job post.AddWork locationIn personPI208244489
