LanceSoft is seeking a travel nurse RN Acute Care Case
Management for a travel nursing job in Boise, Idaho. Job
Description & Requirements
Roles and Responsibilities:
Provide comprehensive case management services for acute care
patients, ensuring safe and timely discharge planning.
Collaborate with interdisciplinary teams including physicians,
social workers, and specialists to create individualized care
plans.
Conduct utilization reviews to ensure appropriate level of care and
alignment with insurance and hospital standards.
Monitor patient progress and intervene as necessary to support
recovery and minimize readmissions.
Use the EPIC EMR system to document assessments, care plans, and
discharge follow-ups accurately.
Maintain up-to-date knowledge of community resources, care options,
and regulations impacting transitions of care.
Advocate for patient needs while maintaining compliance with
healthcare laws and hospital policies.
Requirements:
Experience:
Minimum 5 years of nursing experience with a strong foundation in
Case Management and/or Utilization Review - Required
Education:
Graduate of an accredited nursing program - Required
BSN preferred but not mandatory
Licensure:
Active Idaho RN License or Compact License - Required
Certifications:
BLS (AHA) - Required
Skills:
Advanced knowledge in acute care case management
Familiarity with EPIC charting system
Strong communication, time management, and collaborative skills
Ability to manage large patient caseloads effectively and
independently
